Two-Drug Therapy Helped Kids With Type 2 Diabetes

SUNDAY, April 29 (HealthDay News) -- Children with type 2 diabetes may achieve better blood sugar control with a combination of two drugs, metformin and Avandia, than with metformin alone, a new study suggests.

However, Avandia (rosiglitazone) was recently linked to an increased risk of heart attack and stroke, so it may not be the best drug for these young patients, experts say.

"Many kids with type 2 have a rapidly progressive disease requiring early onset of insulin therapy, and current approaches to oral therapy may be inadequate," said lead researcher Dr. Philip Zeitler, a professor of medicine at the University of Colorado, Denver.

Zeitler noted that the choice of Avandia as a companion medication was made in 2002, before the cardiac problems with the drug were known.

"Given the problems with rosiglitazone, we are not recommending it at this time," he said. "However, no problems with rosiglitazone have been noted in [the study participants] to date, though the cohort size is too small for a thorough safety analysis."

The report was published online April 29 in the New England Journal of Medicine, to coincide with a planned presentation of the finding at the Pediatric Academic Societies' annual meeting in Boston. The National Institute of Diabetes and Digestive and Kidney Diseases funded the research.

For the study, almost 700 obese children, aged 10 to 17, with type 2 diabetes were randomly assigned to metformin alone, metformin plus Avandia or metformin along with intensive lifestyle changes in diet and exercise.

Over the course of 46 months, the researchers found that metformin alone did not adequately control blood sugar levels in 51.7 percent of patients. Among those who received metformin and lifestyle changes, 46.6 percent did not have their blood sugar controlled.

However, among those taking metformin and Avandia, blood sugar levels were not well-controlled in only 38.6 percent of patients, the researchers found.

In addition, blood sugar control was harder to achieve in black and Hispanic children, they added.

"Metformin is probably not as effective as we've assumed, and additional treatment approaches are urgently needed in this population of adolescents with a steadily progressive form of diabetes," Zeitler said.

In terms of lifestyle changes, Zeitler isn't sure why they didn't work better. Part of the reason may be tied to children's reluctance to adopt new diet and exercise habits, he said.

Dr. Joel Zonszein, director of the clinical diabetes center at Montefiore Medical Center in New York City, said that "type 2 diabetes in children and adolescents is a calamity, because we don't have any good medications."

Type 2 diabetes in children is particularly aggressive and can lead to heart and liver problems at a very young age, he said.

"It is not surprising that combination is better than mono-therapy," Zonszein said. "We want to be aggressive in treating type 2 diabetes in children, but we have to balance the risk and benefits of these drugs. But, we really don't have good data and good medications to treat children."

Another expert, Dr. Spyros Mezitis, an endocrinologist at Lenox Hill Hospital in New York City, added that "we need a whole new set of studies to see how to treat type 2 diabetes in children."

Dr. David B. Allen, from the department of pediatrics at the University of Wisconsin School of Medicine and Public Health in Madison, said that "calories consumed in excess of expended is leading to an epidemic of early-life type 2 diabetes, the burden of which is falling disproportionately on disadvantaged youth."

This latest study shows that, once children develop the condition, it is very difficult to prevent deterioration in spite of intensive lifestyle changes or medications, he said.

Barnes & Noble (BKS) shares surged 62 percent Monday on news that Microsoft is investing $300 million in a new venture that will include the bookstore chain's digital and college businesses. Barnes & Noble shares were up $8.52 at $22.20 in heavy midday trading. Microsoft shares (MSFT) were unchanged at $31.98.

Among other big gainers Monday was Sunoco (SUN), up nearly 20 percent at $48.88 after Energy Transfer Partners announced a plan to buy the oil refiner for $5.3 billion. Energy Transfer Partners (ETP) was up about 3 percent.

In other deal-related trading, Gen-Probe Inc. (GPRO) was up 19 percent at $81.61, while?Hologic (HOLX) was down about 10 percent at $19.16. Hologic, a maker of medical devices, announced plans to buy Gen-Probe, a diagnostic test maker, for about $3.72 billion in cash.

Exceed Company (EDS), a Chinese apparel maker, was up 65 percent at $3.45 a share after posting 2011 earnings. Cleantech Solutions International (CLNT), another Chinese company, was up about 25 percent at $5.66.

Demand Media (DMD), an online content creation company that went public earlier this year, was up 23 percent at $8.91.?

Among Monday's losers was Verifone Systems (PAY), down about 10 percent. The maker of credit card swipe systems was downgraded by Deutsche Bank, which questions the company's growth projections, according to Reuters.

Tenneco (TEN) also was down about 10 percent after the auto parts maker posted a decline in earnings.

The Dow Jones industrial average was down 40 points at midday, and broader indexes also were lower.

BEIJING ? A blind legal activist who fled house arrest in his Chinese village is under the protection of American officials, overseas activists said Saturday, putting the U.S. in a difficult position days ahead of a visit by Secretary of State Hillary Rodham Clinton. Chen Guangcheng, who has exposed forced abortions and sterilizations in villages as a result of China's one-child policy, escaped a week ago from his guarded home in Shandong province in eastern China. Chinese-based activists say he was driven away by supporters and then handed over to others who brought him to Beijing. The U.S. and Chinese governments have not confirmed reports that he sought protection at the U.S. Embassy in Beijing. A Texas-based activist group that has been active in promoting Chen's case said that China and the U.S. were in high-level communication about the 40-year-old. "Chen is under U.S. protection and high-level talks are currently under way between U.S. and Chinese officials regarding Chen's status," said a statement from the ChinaAid Association. It cited a source close to the situation. Chen's whereabouts could be a major political complication for the two countries, with Clinton and other top U.S. officials due to arrive in China for the latest round of the Strategic and Economic Dialogue that begins Thursday. The U.S. Embassy in Beijing declined to comment Saturday, as have U.S. officials in Washington. Chinese Vice Foreign Minister Cui Tiankai told a briefing earlier Saturday on the upcoming talks with the U.S. that he had no information on Chen's case. "Your question does not come within the scope of today's briefing. So I have no information to give you," he said when asked about Chen. ChinaAid's founder, Bob Fu, said Chen's case was a benchmark for the United States and its human rights image around the world. "Because of Chen's wide popularity, the Obama administration must stand firmly with him or risk losing credibility as a defender of freedom and the rule of law," he said in the statement. "If there is a reason why Chinese dissidents revere the U.S., it is for a moment like this," Fu said. But the case comes as the United States is looking for help from China on many issues around the world, such as trying to restrain North Korea and Iran on their nuclear ambitions, and push Syria to observe a cease-fire in the fighting in that country. Bilateral disputes over trade, China's currency and U.S. relations with Taiwan are also issues that likely will be part of the talks scheduled for Thursday and Friday. Fu and Chinese-based activists say Chen slipped away from his intensely guarded home on the night of April 22. His wife and 6-year-old daughter are still there. Chen recorded a video as a direct address to Premier Wen Jiabao, condemning the treatment of him and his family and accusing local Communist Party officials by name. Activists sent the video Friday to the overseas Chinese news site Boxun.com, which posted part of it on YouTube. Activist Hu Jia met with Chen after his escape and said the people with Chen later called him. "They said, 'He is in a 100 percent safe place,'" Hu said. "If they say that, I know where that place is. There's only one 100 percent (safe) place in China, and that's the U.S. Embassy." Claims of Chen's location could not be verified. Hu's wife, Zeng Jinyan, posted a photo Friday on Twitter of Chen and Hu together. Chen is wearing the same clothes he wore in the video. Both men are smiling. Chen's escape, if ultimately successful, would boost a beleaguered civil rights community, which has faced rising arrests and other harassment over the past year. If Chen is in the U.S. Embassy or with U.S. officials at another location, it is not known how he would be able to leave or where he could go without Chinese permission. There was no extra security outside the U.S. embassy in Beijing on Saturday. In 1989, when Fang Lizhi, whose speeches inspired student protesters throughout the 1980s, fled with his wife to the U.S. Embassy after China's 1989 military crackdown on the pro-democracy movement, he was forced to stay there for 13 months while the countries discussed his fate. Chen's case is more complicated because his wife and daughter are still trapped in Shandong. Fu said Chen's case should be handled through negotiations, like Fang's, and that his family should not suffer any reprisals. "The odds are that it will be a long negotiation and a stalemate for a time," said Jerome Cohen, a Chinese law expert at New York University. "China has some cards to play starting with Chen's wife and children plus the accomplices to his escape." China's media have been silent on the case, and most words related to Chen and his village have been blocked online. Chinese political analysts have declined to comment. A self-taught lawyer blinded by fever in infancy, Chen served four years in prison for exposing forced abortions and sterilizations in his and surrounding villages. Since his release in September 2010, local officials confined him to his home, despite the lack of legal grounds for doing so, beating him up on several occasions. Chen was widely admired by rights activists at home who last year campaigned to publicize his case among ordinary Chinese and encourage them to go to Dongshigu village and break the security cordon. Even Hollywood actor Christian Bale tried to visit, but as with many others he was roughed up by locals paid to keep outsiders away. Amnesty International and other human rights groups called on the Chinese government to ensure the safety of Chen and his family, saying they had been abused during 18 months of illegal house arrest.

After predicting profits of 5.8 trillion won it managed to top that, notching an operating profit of 5.85 trillion won ($5.16 billion US) for the quarter, a 98 percent gain over a year ago. Phones accounted for 73 percent of the profit, contributing 4.27 trillion won to the bottom line. As the world awaits the debut of what we assume will be the Samsung Galaxy S III May 3rd powered by its Exynos 4 Quad CPU, there's clearly no shortage of demand for the Galaxy S II and Note. Sales of chips and TVs decreased from last quarter, but like its competitor LG, growing sales of high res tablet panels (we wonder which one that might be), 3DTVs and OLEDs increased profitability. Specifically, the high end 7000/8000 series of HDTVs increased sales by 50 percent from last year, while the company plans to focus on "region-specific" LED models for emerging markets, and high end (and high priced) flat-panels for developed markets.

Texas Insider ? Justices Suggest Arizona is Helping on Immigration

The Supreme Court took a dim view of the Obama administration?s effort to halt Arizona?s immigration-crackdown law, with the justices signaling an inclination during oral arguments Wednesday to approve requiring police to check the status of those suspected of being in the U.S. illegally.

At stake is not only Arizona?s law but those in a handful of other states that followed Arizona?s lead over the past two years ? all of which have been put at least partly on hold as challenges wind their way through the federal courts.

While the justices seemed inclined to let the police immigration checks stand ? as long as they don?t substantially increase the amount of time citizens and legal immigrants are detained ? the fate of the other contentious provisions was less clear.

At root, the battle came down to whether Arizona is trying to help or hinder the federal government?s immigration policy.

?The framers vested in the national government the authority over immigration because they understood that the way this nation treats citizens of other countries is a vital aspect of our foreign relations,? Solicitor General Donald B. Verrilli Jr., the government?s top constitutional lawyer, told the court.

But several justices said when it came to the immigration checks, it appeared Arizona was only trying to help.

?It seems to me the federal government just doesn?t want to know who?s here illegally,? Chief Justice John G. Roberts Jr. said at one point.

Federal law already lets local police call and check immigration status ? and in some instances even requires it for prisons and jails. Mr. Verrilli said immigration-status checks are fine as long as police perform them voluntarily, but he said Arizona crossed a line by making it mandatory.

The justices on both sides of the court?s ideological divide didn?t buy that.

?You can see it?s not selling very well ? why don?t you try to come up with something else?? said Justice Sonia Sotomayor, appointed by President Obama.

In addition to the immigration checks, the Arizona law ? known as S.B. 1070 ? had three other provisions blocked by lower courts: giving police the power to arrest anyone they think committed a crime that would get them deported, making it a state crime for an illegal immigrant to apply for a job, and requiring legal immigrants to carry their papers with them at all times.

The Obama administration was on its firmest ground when it argued that Arizona went too far in making it illegal for an illegal immigrant to apply for a job.

Federal law chiefly targets employers, not employees, and Mr. Verrilli said adding stiffer penalties at the state level is not coordination. He said Congress? 1986 immigration law laying out legal penalties was meant to be a comprehensive scheme, and Congress left employees untouched.

Justice Sotomayor seemed to agree.

?It seems odd to think the federal government is deciding on employer sanctions and has unconsciously decided not to punish employees,? she told Paul D. Clement, who argued the case on behalf of Arizona.

A decision is expected before the end of the court?s term in June.

Just eight justices were present for the arguments. Justice Elena Kagan recused herself from the case, presumably because she was the Obama administration?s solicitor general in 2010, when the law was being debated in Arizona.

In one pointed exchange, Justice Antonin Scalia said Arizona?s law doesn?t target anyone the federal government hasn?t already decided shouldn?t be in the U.S.

He said the administration is trying to force Arizona to heel to the administration?s own priorities ? in which Mr. Obama has called for deporting some categories of illegal immigrants while leaving others untouched ? rather than to what U.S. law actually says.

He repeatedly peppered Mr. Verrilli with questions about whether there was another example where the courts had deferred to an administration?s priorities rather than the law.

?I think that?s an extraordinary basis for saying that the state is pre-empted,? Justice Scalia said. ?It?s not criminalizing anything that isn?t criminal under federal law.?

Mr. Verrilli struggled to come up with an answer but said the state?s intent behind the law was the problem.

?The point of this provision is to drive unlawfully present people out of the state of Arizona,? he said.

On that he got no argument from Cochise County Sheriff Larry A. Dever, whose jurisdiction includes 83 miles of U.S.-Mexico border. Speaking to The Washington Times after attending the oral arguments, Sheriff Dever said the reason illegal immigration is swamping Arizona now is because federal government policies pushed it away from California and Texas and into his state in the 1990s.

He said S.B. 1070 doesn?t have a major impact on his county ? the county is close enough to the border that it already can turn over illegal immigrants to the U.S. Border Patrol for removal ? but he said counties farther north will clamp down, which should relieve the flow of people crossing through his county.

?It was moved elsewhere, from San Diego and El Paso, into southern Arizona. The reason it moves elsewhere is because it can,? he said. ?That?s our whole argument. We want the federal government to take up the cause and have the resources it needs to keep it from moving from one place to another.?

Sheriff Dever said Homeland Security officials invite state and local police cooperation in every area of anti-terrorism enforcement except immigration law.

?Why is that?? he asked, adding that the same smugglers who guide people across the border also traffic in drugs and could potentially aid terrorists.
